Licensed Clinical Social Worker

Join our team today as a Licensed Clinical Social Worker where you will provide excellent counseling and psychotherapy services addressing a broad range of behavioral health issues.

Applicants must have:

  • Master’s Degree in an approved Human Services field
  • LCSW Licensure in State of Wisconsin
  • Completed supervised clinical experience of at least 3000 hours in a Behavioral Health capacity
  • Knowledge and experience using DSM-5
  • Substance Abuse Counselor certification is preferred
  • Knowledge of regulations and procedures to administer quality patient care
  • Ability to organize and coordinate patient care with other healthcare agencies in Jackson county
  • Previous experience working with children, adolescents, and families with diverse social and behavioral challenges

Responsibilities of Licensed Clinical Social Worker:

  • Provides short & long-term counseling services with individuals, families and/or groups in the communities we serve
  • Complete intake and assessments to diagnose behavioral health patients
  • Identifies and evaluates a patients strengths, weaknesses, problems and needs for the development of the treatment plan and provides quality documentation
  • Maintains the integrity of highly confidential patient and clinic information
  • Evaluate crisis situations and apply appropriate interventions
  • Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management and patient education to improve self-management of chronic disease
  • Continuously reviews and updates treatment plans throughout the patient’s involvement in treatment
  • Monitor the site’s behavioral health program, identifying problems related to patient services and making recommendations for improvement
  • Maintains complete and current patient records in Electronic Medical Record System (EMR) and abide by Krohn Clinic policies and regulations
  • Documents patient achievement or modification of goals in response to treatment received
  • Evaluate patient needs and determine if referrals to other programs or facilities is necessary
  • Follow up on referrals and patient progress
  • Other job-related duties as assigned
